Patient Forms

Upon entering our office, one of our Chiropractic Assistants will welcome you and get you started right away.  At this time we will request that you complete our new patient forms if you have not done so already.  The new patient forms required are avaiable in the New Patient Center of our website; if you can print and fill out prior to your visit it will save you some time. This paperwork provides us with your health history and information on your condition.

 Examination

Our Technical Chiropractic Assistant will perform a complete chiropractic examination testing your balance, posture and mobility as well as provide a thermal scan on your neck. Other postural and physical tests may also be performed. These tests will help identify any areas of nerve interference which can affect the functions of your body and thus affecting your overall health.

X-Ray Studies

We will take X-rays depending on your specific condition. X-rays help us develop the most effective care for you, and alert us of any serious spinal conditions you may have. Many people can even identify their mis-alignments and degeneration themselves during the review process and are amazed at what they see.

Consultation

Next, you will have a consultation with Dr. Chad to discuss our office, how chiropractic works and what we may be able to do for your health-related problems and concerns.  This initial visit is designed for Dr. Chad to learn more about you, your condition, and expectations to determine how chiropractic care can meet your goals.


Spinal Care

At the conclusion of your findings you will receive your first spinal adjustment. The doctor will explain to you in detail what a spinal adjustment is and how that adjustment affects the health of your spine and nervous system.



Report of Findings

Once the information is collected and examinations are performed, Dr. Chad will give you a detailed report of all findings and answer any questions including:

  1. How can chiropractic help me?
  2. How often do I need to come in?
  3. How much will care cost?

After reviewing your health history, goals, and examining your spine and X-rays, Dr. Chad will discuss recommendations and notify you if your condition requires care with other providers.  Dr. Chad will also provide the best care and wellness program for your needs.

Wellness Program

The doctor may suggest a wellness program to incorporate outside of your spinal care.  If you are in pain when you first come into our office this may include: ice or heat application instructions, certain activities or positions to avoid, and at home exercises and/or stretches.  If you desire, our wellness team will also work with you to create healthy habits and routines for your lifestyle.
